
What if the end of life becomes the ideal starting point for the radical change that is needed for humans to experience a healthy and happy life, from birth to death?
This was the question that New Health Foundation, an award-winning non-profit institution in Spain decided to answer. Thanks to the development of proprietary certification-based change management methodology, it was possible to transform care for people with advanced chronic conditions who require palliative care, and their loved ones, by assessing, monitoring, aligning and optimizing healthcare and social services, while aligning them with community-based assets and reducing costs by more than 40% within a year. This work led to the 2015 Palliative Care Policy Development Award, received during the 14th Palliative Care World Congress, held in Copenhagen, Denmark, as recognition of the importance of the compassionate yet pragmatic model promoted by the Foundation.
